Harrogate and District NHS Foundation Trust is committed to driving forward improvements in the quality of services to improve patient safety, outcomes and experience for people who use our services. Our Board is fully engaged with patient safety and works to ensure a leadership culture which promotes quality and patient safety, and an environment where continuous improvement in harm reduction becomes routine throughout the organisation.
We encourage staff to report incidents, errors or safety concerns because organisations can’t learn and improve unless they know what the problems are.
We investigate incidents and respond promptly to the findings and recommendations of our investigations.
Staff are encouraged participate in “patient safety visits” when members of the Trust Board and Council of Governors visit services to listen to staff talk about any safety concerns.
We encourage “small step change” – putting small ideas to improve safety into practice, and sharing those that work.
We discuss patient safety at all levels in our hospital- starting with the Board of Directors.
A national campaign called Sign up to Safety was launched during 2014. This asked individuals and organisations to commit to the five Sign up to Safety pledges and to produce a 3-5 year safety improvement plan.
Our safety improvement plan was focused on “Effective team working and improved patient safety culture within Maternity” and we were successful in being awarded £23,100 from the NHS Litigation Authority (LA) in relation to Sign up to Safety.

The Trust has agreed to share regular updates on the monitoring of progress, and any safety and learning themes with external partners and directly with the Safety and Learning team at the NHS LA and others e.g. Royal Colleges. The Trust has also agreed to ‘buddy’ with an unsuccessful bidder in terms of sharing best practice to support quality improvements, and to coordinate with other Trusts requesting the same training to ensure procurement benefits from economies of scale and value for money.
