The Trust will send a letter to your GP when you are discharged so they know why you were admitted, what treatment you received and have information about your medication.
The receptionist on the hospital ward or your GP can provide you with a copy of this letter of you want it for your own records.
Your GP will be able to help if you have any issues regarding your treatment or recovery once you have been discharged. You can also visit the NHS Direct website or ring them 24 hours a day, 365 days a year. Just call 0845 4647
