Why Are We Implementing a Proactive Health Coaching Service?

National emergency care admissions to hospital continue to grow, a number these could have been avoided. These attendances often occur where patients feel anxious about their health and are unaware of the alternative support mechanisms that are in place. To help support this group of patients, the Yorkshire and Humber AHSN (YHAHSN) has been successful in obtaining funding from NHSx to support a project to implement a proactive health coaching service commissioned through Health Navigator in the Harrogate and District area for 12 months. North Yorkshire CCG and Harrogate and District NHS Foundation Trust are working in partnership to implement this service.

What is Clinical Coaching?

Clinical Coaching is a telephone-based health coaching service, which is being introduced in the Harrogate and District area to help support patients and assist them in managing their conditions. The service is delivered by registered nurses and healthcare professionals employed by Health Navigator, and is designed to offer support whilst empowering patients to take control of their health. Time can be spent teaching, supporting patients to develop self-care skills, motivating, helping with establishing new healthcare contacts, appointment booking, and contacting their GP and signposting to other supporting services and activities.

What Are the Benefits of Proactive Clinical Coaching?

Patient feedback has shown that clinical coaching can make a real difference to patients’ lives and improve patients’ confidence and ability to manage their own health and healthcare increased following health coaching.

We have had very positive feedback about the clinical coaching calls from patients, which are tailored to meet the needs of each individual.

How are potential patients identified?

Patients who may be at risk of increased unplanned care attendances who may benefit from clinical coaching are identified through data analytics. The approach combines algorithms along with manual screening to ensure that appropriate clinical coaching candidates are identified.

All patient data used in the algorithm is pseudonymised i.e. personal identifiable details are replaced with a reference number.
Once patients have been identified as potential candidates for Clinical Coaching by the algorithm, their details are shared with the clinical team at HN. This team will review the patient’s health record to check suitability before the patient is invited to take part in the service.

Who are Health Navigator?

HN is comprised of professionals from a range of clinical, digital, data and business backgrounds. We are highly experienced in delivery of innovative clinical services, use of applied AI within healthcare, research and evaluation.

Most of the HN team have worked within the NHS so are familiar with its processes and all the patient facing services they deliver are by trained and qualified healthcare professionals. They have an excellent Advisory Board and Regional Advisers who are senior stakeholders from within NHS commissioning, primary care, population health management, adult social care and research, that they actively work with to achieve their goals.

HN won an award in the Self-Management Category at the Medipex NHS Innovation Awards and Showcase and was also shortlisted as a finalist in the HSJ Value in Healthcare Awards 2017.
