Community Integrated Care, incorporating District Nursing and Therapy, is a universal service. It can be accessed by adults over 18 years old, who are housebound or require treatment at home.
Each team covers a geographical area and we work closely with GPs to provide community care with a multi-disciplinary team approach. The Community Care teams work out of four locations across the local area: Knaresborough, Ripon and two teams in Harrogate.
We provide long and short term nursing interventions, rehabilitation and support the management of long term conditions. We also provide an important role in keeping hospital admissions and readmissions to a minimum and ensuring that patients can return to their own homes as soon as possible after a hospital stay. We encourage, support and facilitate self management and patient goal setting where possible.
We provide nursing and therapy interventions in a wide variety of conditions including:
- Diabetes
- Complex wounds and leg ulcers
- Post-operative wounds
- Pressure ulcers
- Palliative and end of life care
- Neurological conditions
- Frequent falls, immobility
- Continence
We also offer assistance in the assessment and supply of equipment.

Referrals
We accept referrals from a range of sources including GPs, hospitals, voluntary agencies, carers and families, social services, private care agencies, and residential homes. Please note that ALL referrals need to go through our Single Point of Referral telephone number: 01423 554632.
