To help us support patients who are currently listed for surgery at Harrogate District NHS Foundation Trust in preparing for their operation, we ask that they complete a pre-operative health questionnaire online using our patient portal, Patient Knows Best (PKB).
Completing this questionnaire will help us provide you with the best possible care and make your surgical experience as smooth as possible. Depending on the answers you provide and the type of surgery you are having, you might be invited for further assessments or tests at Harrogate District Hospital.
